MonmouthCares is one of fifteen Care Management Organizations in New Jersey Statewide Children’s System of Care. As the Care Management Organization (CMO) for Monmouth County, New Jersey it has the responsibility for face-to-face care coordination, comprehensive service planning for children, youth and their families who have complex challenges/needs. MonmouthCares continues to expand its support for local community resource development. This 1-hour/30-minute session will be held live at MonmouthCares, along with the panel there will be an audience of their staff for an exciting conversation. Attendees will be given a glimpse into the work being done every day of Care Management and have the amazing opportunity to ask questions. The areas of discussion will include the history of MonmouthCares, Wraparound/Child Family Teams, and the use of TCOM Tools.
