Cardiovascular, renal and metabolic (CRM) conditions are the leading cause of death worldwide, accounting for up to 20 million deaths per year. While some progress has been made for people living with heart disease, many governments and healthcare systems are still missing out on a key opportunity – interconnected care and support for people living with CRM conditions.
